Long Dan Er Miao San is a classical formula traditionally used to clear severe Damp-Heat and Liver Fire associated with acute, intensely inflamed skin and ear disease.

In veterinary practice, it is primarily used in animals showing marked redness, Heat, irritation, swelling, discharge or moist inflammatory lesions. These patients are often highly reactive, uncomfortable and acutely inflamed.

This formula is particularly useful in severe allergic dermatitis, moist pyoderma, acute bacterial skin inflammation and intensely inflamed otitis externa where inflammatory Heat and Fire are pronounced.

Currently in Australia, KAN herbal blends are only allowed to be dispensed by a registered veterinarian.

As with all Traditional Chinese Herbal blends, KAN Herbs are best prescribed by an integrative veterinarian who can examine your animal in person. Telehealth consultations however can be used when access to an appropriate veterinarian is difficult or not possible.

How it Works

Long Dan Er Miao San combines elements of Long Dan Xie Gan Tang with Er Miao San to strongly clear Damp-Heat and Liver Fire affecting the skin and lower body.

In Traditional Chinese Medicine, severe Damp-Heat and Fire patterns may present with acute redness, swelling, irritation, exudation and rapidly escalating inflammation. These patterns are often associated with intense Heat, inflammatory discharge and highly reactive skin disease.

Clinically, this formula is commonly used in:

  • acute allergic dermatitis
  • moist pyoderma
  • acute bacterial skin inflammation
  • intensely inflamed otitis externa
  • hot, inflamed ears or paws
  • rapidly developing red, moist or exudative skin lesions

This is a very cooling formula and is generally intended for short-term use during severe inflammatory flare states rather than long-standing chronic disease. Despite the inclusion of Phellodendron and Cang Zhu to help protect digestive function, some animals may still develop reduced appetite or small bowel diarrhoea during treatment.

Animals suited to this formula often show:

  • intensely red or moist tongue
  • rapid or slippery pulse
  • Heat intolerance
  • irritability or agitation
  • moist, tacky or exudative lesions
  • severe redness and inflammatory discharge

Think: severe acute Damp-Heat and Fire driving intensely inflamed skin and ear disease.

This formula is generally less appropriate where:

  • deficiency and weakness predominate
  • the patient is cold, depleted or exhausted
  • inflammation is chronic but not intensely hot
  • dryness outweighs Damp and inflammatory discharge

Compared with Si Miao San, this formula is more aggressively Heat-clearing and is designed for severe acute inflammatory flare states rather than broader metabolic Damp-Heat patterns.

How is Long Dan Er Miao San different from Si Miao San?

Both formulas address Damp-Heat patterns, however Long Dan Er Miao San is significantly stronger and more aggressively Heat-clearing.

It is generally used in severe acute inflammatory skin and ear disease characterised by intense redness, Heat, irritation and discharge, while Si Miao San is more commonly used for broader Damp-Heat and metabolic inflammatory patterns.

Why do some skin conditions suddenly become intensely red, hot or moist?


In Traditional Chinese Medicine, Damp-Heat may escalate into more severe inflammatory “Fire” patterns. These animals often develop rapidly worsening redness, swelling, irritation, discharge or moist lesions associated with acute inflammatory flare states.

Can Long Dan Er Miao San be used alongside steroids or conventional medication?

Yes. In integrative veterinary practice, this formula is commonly used alongside conventional therapies during severe inflammatory skin or ear flare states.

Because this is a strong Heat-clearing formula, veterinary supervision is recommended, particularly in animals receiving multiple medications or with underlying gastrointestinal sensitivity.

Why might this formula reduce appetite or cause diarrhoea?


This is a strongly cooling formula designed for severe inflammatory Heat conditions. In some animals, particularly those with weaker digestion or less intense Heat, the cooling nature of the formula may contribute to reduced appetite or small bowel diarrhoea during treatment.
